A 30-year-old worker entered a water tank to fix a leaking valve, unaware of the dangers that lay ahead. As the worker began the repair, he suddenly lost consciousness due to asphyxiation caused by hazardous air conditions inside the tank. Two of his co-workers, noticing something was wrong, bravely entered the tank in an attempt to rescue him. Unfortunately, they were also affected by the poor air quality, but thankfully, they did not sustain any serious injuries.
